Omega-3 (EPA + DHA): Mechanism, Dosing, and Evidence

Introduction

Omega-3 fatty acids — specifically eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) — are essential polyunsaturated fats that the human body cannot synthesize efficiently. While alpha-linolenic acid (ALA) from plant sources can theoretically convert to EPA and DHA, the conversion rate is dismally low: less than 5% for EPA and less than 0.5% for DHA. This makes direct supplementation with preformed EPA and DHA from marine sources the only reliable strategy for achieving therapeutic levels.

EPA and DHA serve fundamentally different roles. EPA is the anti-inflammatory powerhouse, serving as the precursor to resolvins and protectins that actively resolve inflammation. DHA is the structural backbone of neuronal membranes — approximately 60% of the brain's fatty acid content is DHA. Together, they represent one of the most well-researched supplement categories in all of nutrition science.

Mechanism of Action

EPA: Inflammation Resolution

EPA is enzymatically converted by cyclooxygenase and lipoxygenase enzymes into specialized pro-resolving mediators (SPMs): E-series resolvins, protectins, and maresins. Unlike NSAIDs that merely suppress inflammation, SPMs actively resolve it — clearing inflammatory debris, promoting macrophage phagocytosis, and restoring tissue homeostasis. EPA also competes with arachidonic acid (AA) for COX-2, shifting the eicosanoid profile from pro-inflammatory prostaglandins (PGE2) toward anti-inflammatory prostaglandins (PGE3).

DHA constitutes the primary structural fatty acid of brain gray matter and retinal photoreceptors. It confers unique biophysical properties to neuronal membranes: increased fluidity, enhanced receptor trafficking, and faster synaptic vesicle fusion. DHA-enriched membranes support optimal function of embedded proteins including serotonin receptors, dopamine receptors, and ion channels critical for neurotransmission. DHA deficiency reduces brain-derived neurotrophic factor (BDNF), impairing neuroplasticity and memory consolidation.

Triglyceride Form Superiority

Fish oil exists in two primary forms: triglyceride (TG) and ethyl ester (EE). The triglyceride form has 70% higher bioavailability than ethyl ester because pancreatic lipase cleaves TG bonds 10-50x faster than EE bonds. Recent Research (2025–2026)

The 2026 PISCES trial found that 4 g/day of fish oil in dialysis patients reduced major cardiovascular events by 43% (heart attacks, strokes, cardiac-related deaths, and vascular amputations) compared to placebo. A 2025 systematic review and meta-analysis of 42 studies (176,253 participants) confirmed that EPA and DHA modulate lipid metabolism, inflammation, platelet and endothelial function, the gut-heart axis, ion channels, and autonomic function via vagal tone. A 2025 dose-response meta-analysis of 23 RCTs (2,061 patients with coronary heart disease) assessed the impact of omega-3 on lipid profiles and arterial plaque volume. However, 2025 research also found a nuanced risk: a meta-analysis of 8 RCTs (83,112 participants) showed that EPA/DHA supplementation was linked to a 24% higher relative risk of atrial fibrillation, though newer analyses measuring circulating omega-3 levels show higher blood concentrations are associated with lower AF risk.
