Introduction

Biotin (vitamin B7, formerly vitamin H) is a water-soluble B vitamin that serves as a critical cofactor for five carboxylase enzymes involved in fatty acid synthesis, amino acid catabolism, and gluconeogenesis. It is perhaps best known for its role in keratin production — the structural protein that forms hair, skin, and nails. While overt biotin deficiency is rare, suboptimal levels are common, particularly in individuals with high metabolic demands, those taking certain medications (anticonvulsants, antibiotics), or during pregnancy. At supplemental doses of 10,000 mcg, biotin provides supraphysiological support for keratin infrastructure, but comes with a critical caveat: it interferes with streptavidin-biotin immunoassays used in many common blood tests.

Mechanism of Action

Keratin Infrastructure

Biotin is a prosthetic group for acetyl-CoA carboxylase (ACC), which catalyzes the first committed step in fatty acid synthesis. Fatty acids are essential building blocks for the lipid matrix that surrounds and protects keratin fibers in hair, skin, and nails. Biotin also supports the production of keratin itself through gene regulation — it activates biotinylation of histones, an epigenetic modification that influences the expression of genes encoding keratin and keratin-associated proteins. This dual action (structural lipid synthesis + gene regulation of keratin production) explains why biotin supplementation improves hair thickness, nail hardness, and skin texture.

Fatty Acid Synthesis and Gluconeogenesis

Beyond keratin, biotin is a cofactor for pyruvate carboxylase (gluconeogenesis), propionyl-CoA carboxylase (odd-chain fatty acid and amino acid metabolism), and 3-methylcrotonyl-CoA carboxylase (leucine catabolism). These enzymes are essential for energy production, amino acid turnover, and maintaining blood glucose levels during fasting. Biotin deficiency impairs all of these pathways simultaneously.

CRITICAL: Lab interference warning. High-dose biotin (5,000+ mcg) interferes with streptavidin-biotin immunoassays, which are used in many common lab tests. This can cause falsely low TSH (mimicking hyperthyroidism), falsely elevated T4/T3, falsely low troponin (potentially missing a heart attack), and falsely positive/negative HCG. You MUST stop biotin supplementation at least 48 hours before any blood draw. The FDA issued a safety communication about this in 2017.
